The W3 Total Cache plugin for WordPress is a tool that helps make websites faster and more secure. Unfortunately, versions up to 0.9.4.1 of this plugin are vulnerable to an authorization bypass. This means that in certain circumstances it may be possible for an attacker to bypass security measures that are put in place to protect the website. If successful, this can lead to the OPCache being emptied. To avoid this, it is important to ensure that you are running the most up-to-date version of the W3 Total Cache plugin.
